You may fill out the following form online, then print it and sign it. You will then need to take it to the California Highway Patrol office at 995 Fir Street, Chico, for their office to sign off on it before submitting the completed form to our office. You may bring the form in person to the Fair Main Office or send it by US Mail, making certain we receive it before your event.
