You may fill it out online if you wish, then print it out and sign it. Please return it to us in person at the Silver Dollar Fair office or by U.S. Mail no later than February 15, 2008.
Please send the following information: - Detailed list of the items you wish to sell or promote
- Pictures of items you wish to sell or promote (if available)
- Your preference of indoor or outdoor space
- Whether you have a ''pitch'' or microphone setup for your booth
When your application is approved, we will send you a contract and additional information. You will need to provide the Silver Dollar Fair with a Certificate of Insurance for $1,000,000 general liability, listing the Silver Dollar Fair as additional insured for the dates of the fair (as shown on the sample Acord certificate). This insurance can also be purchased through our office for $90. | |

| | Trophies will be presented to the Outstanding Indoor Booth and the Outstanding Outdoor Booth. Booths will be judged on attractiveness of display. | |
